Replacement Birth Certificates

NEW! Newborn Registration Service
Now, without leaving the comfort of your home, busy parents can go online and access the Newborn Registration Service. In one convenient visit:
- Register your baby’s birth
- Apply for a Birth Certificate
- Apply for a Social Insurance Number
- Apply for Canada Child Benefits
The service takes approximately 20 to 25 minutes to complete and is available for babies under the age of one year.
Birth Certificates for Newborns
- If the birth certificate is for a newborn, and you have completed the Statement of Live Birth form, sent it to the Office of the Registrar General, and did not apply for a birth certificate at the same time, you will receive a Notice of Birth Registration in the mail. The Notice confirms that your child's birth has now been registered in Ontario and you can apply for the birth certificate.
- If the birth certificate is for a newborn and you have not completed the Statement of Live Birth, then click below on our Newborn Registration Service. It is the easiest way to register your baby’s birth and the fastest way to apply for his/her birth certificate and Social Insurance Number all at the same time.
First Birth Certificate for a Child over 1 year old
- If the birth certificate is for your child who is over the age of 1 and your child’s birth is not registered, you will need to apply for a birth certificate. Once we have confirmed that the birth is not registered, we will send you the forms to complete a delayed registration of birth. After the birth is registered, the birth certificate will then be issued.

What happens if my child is less than a year old and his/her birth hasn’t been registered and I am required to travel with my newborn. What can I do?
Parents who anticipate a need for travel soon after birth are advised to use the fully electronic 4-in-1 Newborn Registration Service to register the newborn’s birth and apply for a birth certificate at the same time. If you did not order a birth certificate at the same time the birth registration information was submitted, you should apply for a birth certificate via the Office of the Registrar General Premium Online or Emergency services for the quickest delivery.
