Severe Medical Searches (Adopted Person)

If you are an adopted person or the descendant of an adopted person — and you or your descendants have or had a severe mental or physical illness — you can apply for a Severe Medical Search.

This could help you give medical information to your birth relatives. It could also help you get medical information from your birth relatives.

Restrictions

  • Severe Medical Searches have very strict criteria
  • Your doctor or health care professional must complete a section of the application form
  • It must be clear that you, your descendant or your birth relative will get a direct medical benefit from receiving this medical information (A direct medical benefit means that the severe mental or physical illness will likely be easier to diagnose or treat)

Applying for a Severe Medical Search

  • You must be an adopted adult who is at least 18 years old. If you are under 18, you must have the written consent of your adoptive parent or guardian
  • You or one of your descendants must have suffered from, or be suffering from, a severe mental or physical illness
  • This is a free service

  • Send your application by mail, registered mail or courier only
  • We will send a letter of confirmation when we receive your application
