I am a Birth Parent (My Child was Adopted) - Post-Adoption Birth Information
Beginning June 1st, 2009, you can apply for more information from birth and adoption records.
This information is called post-adoption birth information. It includes identifying information, like the name your child was given after being adopted.
Post-adoption birth information services include:
Information from adoption orders and Ontario birth registrations for birth parents
Copies of adoption orders and Ontario birth registrations for adopted adults
Disclosure vetoes for birth parents and adopted adults to protect their privacy. Read the section on disclosure vetoes to learn more
No contact notices for birth parents and adopted adults who don’t want to be contacted by each other. Read the section on notices to learn more
Notices of contact preference for birth parents and adopted adults who know how they want to be contacted. Read the section on notices to learn more
Important Information
Post-adoption birth information will include identifying information, like your name
If you don’t want the adopted person to know your identity, you must file a disclosure veto
If the adoption was finalized before September 1, 2008, you can file a disclosure veto
No contact notices and notices of contact preference can also be filed
Applying for post-adoption birth information
The adopted person must have been born in Ontario and the adoption registered in Ontario
Send your application by mail, registered mail or courier only
Due to a high volume of applications, there is currently no standard processing time and we will not be able to confirm that we received your application

Any applications for disclosure of post-adoption birth information received before June 1st, 2009 will be processed beginning on June 1, 2009

Child Born Outside Ontario
If the adopted person was born outside Ontario, you will need to contact the province where the birth took place
Disclosure Veto Filed
If the adopted person filed a disclosure veto, you will not receive post-adoption birth information
Instead, you’ll get a copy of the disclosure veto including any statement that was filed by the adopted person with the disclosure veto. This statement may include medical or family history and/or the reasons he or she does not want identifying information released
Adopted adults and birth parents have been able to apply for disclosure vetoes since September 1, 2008. Read the section on disclosure vetoes to learn more.
Disclosure vetoes are no longer in effect after the person who filed the veto withdraws it, or after the Office of the Registrar General has evidence that the person who filed the veto has died
No Contact Notice Filed
If the adopted person filed a no contact notice, you must agree in writing not to contact him or her before you can receive your post-adoption birth information
Once you have agreed not to contact him or her, you will receive the post-adoption birth information along with a copy of the no contact notice and any statement which he or she may have filed containing medical or family history and/or reasons for not wanting contact
Notice of Contact Preference Filed
If the adopted person filed a notice of contact preference, you will receive your post-adoption birth information along with a copy of the notice that tells you how he or she would like to be contacted
