I am an Adopted Person (I was Adopted) - Adoption Orders

An adoption order is a court-issued document that makes your adoptive parents your parents for all purposes of the law. You or your adoptive parents may need an adoption order to apply for identification or other documents.

You may apply for a copy of your adoption order at any time. Information that may identify your birth parents will be removed.

If you want to know the name(s) of your birth parent(s), you will need to apply for your post-adoption birth information

Applying for an Adoption Order

  • You must be an adopted adult who is at least 18 years old
  • You must have been adopted in Ontario
  • This is a free service
  • Download the guide and adoption order application, or call 416-325- 8305 or 1-800-461-2156 (TTY 416-325-3408) to get more information
  • Send your application by mail, registered mail or courier only
  • We will not confirm that we received your application
