Health Card Renewal/Re-registration Appointment Information

Before you book an appointment to get your Ontario Health card, read the following to understand what you need to do.


Appointment Service Locations

Currently, health card renewal and re-registration services by appointment are available online at select locations. ServiceOntario is increasing the number of offices where you can book an appointment. To assist you in finding the location nearest you, a mapping feature is available within the appointment booking service.

Types of Appointments

You can book two types of appointments:

  • A Health Card Renewal is a replacement for an expired or soon to expire photo health card
  • A Health Card Re-registration will replace the ‘old’ red and white cards with a ‘new’ photo card

There is no fee for an Ontario Health Card or for booking an appointment using the ServiceOntario Online Booking system.

Required Information for a Health Card Appointment

To book your appointment, you will need the following information:

  • Your Ontario Health Card
  • A printer
  • Adobe Reader

You will need to bring the following in person to your appointment:

  • Existing Health Card
  • Proof of Citizenship
  • Proof of Residency in Ontario
  • Support of Identity
