1. If I have signed my donor card, why do I need to register?

A donor card is a paper card carried by the prospective donor. Your decision to donate is only known to the extent that you share this decision with your family and friends.

However, when you register your consent to donate organs and tissue, this information is stored in a Ministry of Health and Long-Term Care database. The Ministry will disclose information about your decision to Trillium Gift of Life Network, Ontario's organ and tissue donation agency, for the purpose of ensuring that your decision to donate is known and respected. By registering your consent to donate, you ensure that your donation decision is recorded and is made available to the right people at the right time, and is shared with your loved ones.

2. Who can register to be an organ and tissue donor?

  • Anyone 16 years of age or over
  • Anyone in possession of a valid health card number and version code (if applicable)

3. Is there a cost to register my consent online?

No, there is no cost..

4. Is there another way to register other than online?

Yes. You can register your consent in-person at a ServiceOntario centre. You can also go to the Trillium Gift of Life Network website and download and complete a Gift of Life Consent Form. The completed form should be mailed to:

5. How do I know the information I am providing is secure?

The Ontario government has strict privacy standards in place. The privacy statement provides detailed information about how we are protecting your privacy.

9. Why do you need my email address?

This is an optional feature that provides you with immediate confirmation. Your email information is not saved.

10. I experienced a technical problem while using the service.  Should I be concerned about the security of my personal information?

No. Although computer glitches will sometimes occur, your personal information is protected at all times.

11. What do I do if I experience a technical problem and the information I have entered on the form is erased?

Unfortunately, your information is lost and you will have to re-enter the information.

12. When I try to use my browser’s “Back” button, I sometimes get an error message. Why am I not able to go back?

We are protecting the privacy of your personal information by adding a security feature that forbids your browser from holding certain information. If you want to go back to a page you just left, use the "Previous" button located on the page itself.

13. When will my donation decision be accessed by the Trillium Gift of Life Network and shared with my family?

Your decision to consent to donate is stored in a Ministry of Health and Long-Term Care database. The Ministry will use and disclose the information about your decision to the Trillium Gift of Life Network only for the purpose of ensuring that your decision to donate is known.

Your organ and tissue donation decision will only be accessed by the Trillium Gift of Life Network and shared with your family in the event of your death or imminent death when all lifesaving efforts have failed. Your donation decision will not be accessed by Trillium Gift of Life Network at any other time.
