Forms
1. (1) The following forms are prescribed as the required forms for the purposes indicated:
1. Form 1: the notice to the clerk of the local municipality that the owner desires fence-viewers to view and arbitrate under subsection 4 (1) of the Act.
2. Form 2: the notice by the clerk of a municipality or by another designated person to an owner or occupant that he or she shall cause fence-viewers to arbitrate under subsection 4 (2) of the Act.
3. Form 3: the notice given by the clerk of a municipality to fence-viewers that they are required to meet and arbitrate under subsection 4 (2) of the Act.
4. Form 4: the award by fence-viewers under subsection 8 (1) of the Act.
5. Form 5: the notice of appeal served by an owner under subsection 10 (1) of the Act.
6. Form 6: the affidavit of service of a notice of appeal to be filed by an owner under subsection 10 (1) of the Act.
7. Form 7: the notice given by the clerk of a municipality, or another designated person, to an owner or occupant that he or she shall cause fence-viewers to reattend under subsection 11 (8) of the Act.
8. Form 8: the notice given by the clerk of a municipality to fence-viewers that they are required to reattend under subsection 11 (8) of the Act.
9. Form 9: the certificate by fence-viewers under clause 12 (1) (a) of the Act certifying the default of the adjoining owner and the value of the work.
10. Form 10: the certificate by fence-viewers under clause 12 (1) (b) of the Act certifying the default of the adjoining owner and the amount payable by the adjoining owner.
11. Form 11: the notice under subsection 12 (8) of the Act by the treasurer of a municipality to an adjoining owner against whom an award is being enforced that a payment has been made.
12. Form 12: the determination with directions made by fence-viewers under subsection 13 (6) of the Act.
13. Form 13: the decision prepared by fence-viewers under subsection 14 (1) of the Act that no award or no certificate or determination with directions shall be made.
14. Form 14: the agreement in writing between owners under section 16 of the Act.
15. Form 15: the agreement in writing between a municipality or a local board with owners of adjoining land under subsection 23 (3) of the Act.
