1. The office of the Crown to which a copy of a claim for lien must be given under subsection 34 (3) of the Act is as follows:

1. Where the contract is with a Ministry of the Crown, the office of the Director of Legal Services of that Ministry.

2. Where the contract is with the Ontario Mortgage and Housing Corporation, the office of the Director of Legal Services of the Ministry of Housing.

3. Where the contract is with a college of applied arts and technology, the office of the president of the college.

4. Where the contract is with any other office of the Crown, the chief executive officer of that office.  R.R.O. 1990, Reg. 175, s. 1.

2. (1) A notice to a contractor under section 18 of the Act may be in Form 1.  R.R.O. 1990, Reg. 175, s. 2 (1).

(2) A notice to a landlord under subsection 19 (1) of the Act may be in Form 2.  R.R.O. 1990, Reg. 175, s. 2 (2).

(3) A notice to a contractor under subsection 19 (1) of the Act may be in Form 3.  R.R.O. 1990, Reg. 175, s. 2 (3).

(4) A notice to a lien claimant under subsection 19 (3) of the Act may be in Form 4.  R.R.O. 1990, Reg. 175, s. 2 (4).

(5) A declaration of last supply under subsection 31 (5) of the Act shall be in Form 5.  R.R.O. 1990, Reg. 175, s. 2 (5).

(6) A certificate of the substantial performance of a contract under section 32 of the Act shall be in Form 6.  R.R.O. 1990, Reg. 175, s. 2 (6).

(7) A certificate of completion of a subcontract under subsection 33 (1) of the Act shall be in Form 7.  R.R.O. 1990, Reg. 175, s. 2 (7).

(8) A claim for lien under section 34 of the Act shall be in Form 8.  R.R.O. 1990, Reg. 175, s. 2 (8).

(9) Revoked:  O. Reg. 174/11, s. 1.

(10) A certificate of action under section 36 of the Act shall be in Form 10.  R.R.O. 1990, Reg. 175, s. 2 (10).

(11) A notice requiring a lien claimant whose lien is sheltered under the statement of claim of some other person to provide further particulars of the claim or of any fact alleged in the claim under paragraph 4 of subsection 36 (4) of the Act may be in Form 11.  R.R.O. 1990, Reg. 175, s. 2 (11).

(12) A notice under clause 40 (3) (b), (c) or (d) of the Act may be in Form 12.  R.R.O. 1990, Reg. 175, s. 2 (12).

(13) A notice under clause 40 (3) (a) of the Act may be in Form 13.  R.R.O. 1990, Reg. 175, s. 2 (13).

(14) A release of lien under section 41 of the Act shall be in Form 14.  R.R.O. 1990, Reg. 175, s. 2 (14).

(15) A notice of postponement under section 43 of the Act shall be in Form 15.  R.R.O. 1990, Reg. 175, s. 2 (15).

(15.1) A bond posted as security under section 44 of the Act shall be in Form 15.1.  O. Reg. 416/93, s. 1.

(16) A judgement under section 58 of the Act directing a reference of the whole action to a master or appointed local master for trial shall be in Form 16.  R.R.O. 1990, Reg. 175, s. 2 (16).

(17) A notice of trial under section 60 of the Act may be in Form 17.  R.R.O. 1990, Reg. 175, s. 2 (17).

(18) A notice of settlement meeting under section 60 of the Act may be in Form 18.  R.R.O. 1990, Reg. 175, s. 2 (18).

(19) A judgement under section 62 of the Act shall be,

(a) in Form 19, where the lien attaches to the premises; or

(b) in Form 20, where the lien does not attach to the premises.  R.R.O. 1990, Reg. 175, s. 2 (19).

(20) A report under section 62 of the Act shall be,

(a) in Form 21, where the lien attaches to the premises; or

(b) in Form 22, where the lien does not attach to the premises.  R.R.O. 1990, Reg. 175, s. 2 (20).

(21) A financial guarantee bond under subsection 78 (10) of the Act shall be in Form 23.  R.R.O. 1990, Reg. 175, s. 2 (21).

(21.1) A notice of intention to register a condominium under section 33.1 of the Act shall be in Form 24.  O. Reg. 174/11, s. 2.

(22) Where an affidavit provided for in this Regulation is sworn or affirmed by an illiterate person, the jurat to the affidavit shall be in the form prescribed in the Superior Court of Justice rules of court.  R.R.O. 1990, Reg. 175, s. 2 (22); O. Reg. 366/02, s. 1.

3. (1) Copies of certificates of the substantial performance of a contract shall be published in a construction trade newspaper in the following form and manner:

1. In each issue, all certificates shall appear on a single page or consecutive pages, and shall be arranged alphabetically by the upper-tier municipality or single-tier municipality in which the premises are situated.

2. The contents of each certificate of the substantial performance of a contract published in a construction trade newspaper shall be the same as in the original of the certificate, except that the signature of the payment certifier or other persons certifying substantial performance shall be deleted.  R.R.O. 1990, Reg. 175, s. 3 (1); O. Reg. 366/02, s. 2.

(2) Declarations of the substantial performance of a contract shall be published in the same form and manner as certificates of the substantial performance of a contract.  R.R.O. 1990, Reg. 175, s. 3 (2).
