Note: This Regulation was revoked on the day Parts I to XXVII of the Employment Standards Act, 2000 comes into force. This day has been named as September 4, 2001. See: O. Reg. 293/01, ss. 2, 9.

This Regulation is made in English only.

1. (1) An employer who is required to give notice of termination under subsection 57 (2) of the Act shall provide to the Minister,

(a) the information indicated in Form 1; and

(b) a copy of any adjustment plan negotiated under section 41.1 of the Labour Relations Act. O. Reg. 361/94, s. 1.

(2) The employer shall provide the information required under subsection (1) to the Minister by delivering it to the Director of the Employment Practices Branch of the Ministry between 9 a.m. and 5 p.m. from Monday to Friday. O. Reg. 361/94, s. 1.

(3) The employer shall post Part A of Form 1 and the information required by clause (1) (b) in accordance with subsection 57 (4) of the Act. O. Reg. 361/94, s. 1.

2. A notice under subsection 6 (2) of the Act shall be in Form 2. R.R.O. 1990, Reg. 323, s. 2.
