1. For the purposes of subsection 37 (3) of the Act,
(a) the form that is prescribed is the form titled “Functional Abilities Form for Planning Early and Safe Return to Work” that is dated July, 2006, and that is available on the Internet on the website of the Workplace Safety and Insurance Board; and
(b) the information to be given by a health professional is the information required to complete the form referred to in clause (a). O. Reg. 28/07, s. 1; O. Reg. 124/16, s. 1.
