This Regulation is made in English only.

1. For the purposes of subsection 37 (3) of the Act,

(a) the form that is prescribed is the form titled “Functional Abilities Form for Planning Early and Safe Return to Work” that is dated July, 2006, and that is available on the Internet through www.wsib.on.ca/wsib/wsibsite.nsf/public/FormsOverview; and

(b) the information to be given by a health professional is the information required to complete the form referred to in clause (a).  O. Reg. 28/07, s. 1.
