1. Schedule 2 to Ontario Regulation 275/94 is amended by adding the following item:

Human papillomavirus (HPV) vaccine

2. Schedule 3 to the Regulation is amended by adding the following items:

Almotriptan — for renewal only

Beclomethasone dipropionate (topical)

Betahistine dihydrochloride — for renewal only for the treatment of recurrent vertigo associated with Ménière’s disease

Budesonide-formoterol fumarate dihydrate — for renewal only

Ciclesonide — for renewal only for the prophylactic management of steroid-responsive bronchial asthma

Ciclopirox olamine (shampoo)

Clindamycin phosphate and benzoyl peroxide

Clobetasone butyrate

Diflucortolone valerate

Donepezil hydrochloride — for renewal only

Ezetimibe — for renewal only

Galantamine hydrobromide — for renewal only
