Note: Since the end of 2001, Ontario Regulation 201/96 has been amended by Ontario Regulation 64/02.  Previous amendments are listed in the Table of Regulations published in The Ontario Gazette dated January 19, 2002.

1. The definition of “Formulary” in subsection 1 (1) of Ontario Regulation 201/96 is revoked and the following substituted:

“Formulary” means the Ministry of Health and Long-Term Care publication titled “Drug Benefit Formulary/Comparative Drug Index” (No. 37) dated March 7, 2001, including the amendments to the publication dated June 7, 2001, October 11, 2001, April 4, 2002 and July 29, 2002;

2. This Regulation comes into force on July 29, 2002.
