Note: Since the end of 2001, Ontario Regulation 201/96 has been amended by Ontario Regulations 64/02, 214/02, 301/02 and 375/02.  Previous amendments are listed in the Table of Regulations published in The Ontario Gazette dated January 19, 2002.

1. The definition of “Formulary” in subsection 1 (1) of Ontario Regulation 201/96 is revoked and the following substituted:

“Formulary” means the Ministry of Health and Long-Term Care publication titled “Drug Benefit Formulary/Comparative Drug Index” (No. 38) dated January 30, 2003;

2. This Regulation comes into force on January 30, 2003.
