Amending Reg. 641 of R.R.O. 1990

(GENERAL)

1. Subsection 2 (1) of Regulation 641 of the Revised Regulations of Ontario, 1990 is amended by striking out “in accordance with Form 1”.

2. Clause 3 (1) (a) of the Regulation is amended by striking out “in Form 2”.

3. Subsection 5 (1) of the Regulation is amended by striking out “in accordance with Form 2”.

4. Forms 1 and 2 of the Regulation are revoked.

Commencement

5. This Regulation comes into force on the day it is filed.
