• Appointment of Agent for Service (Form 2 under the EPCA)
  Foreign business corporations incorporated outside of Canada who wish to do business in Ontario must appoint an Agent for Service in Ontario. An Agent for Service must be an individual, 18 years of age or older, who is resident in Ontario or a corporation with its head or registered office in Ontario. Form must be completed and filed in duplicate with Companies and Personal Property Security Branch.
• Application for Extra-Provincial Licence (Form 1 under the EPCA)
  Foreign business corporations incorporated outside of Canada must obtain an Extra-Provincial Licence in order to do business in Ontario. Form must be completed and filed in duplicate with Companies and Personal Property Security Branch.

  This form is to be used if a corporation with an extra-provincial licence decides to cease carrying on business within the province of Ontario.

• Articles of Incorporation (Form 1 under the BCA)
  Articles of Incorporation, Form 1 under the Ontario Business Corporations Act (OBCA) must be completed and filed to incorporate a business corporation in Ontario.
• Consent to Act as First Director (Form 2 under the BCA)
  This form must be completed and kept at the corporation's registered office address if the first director of the corporation is not one of the incorporators.
• Articles of Amalgamation (Form 4 under the BCA)
  This form applies to two or more active Ontario business corporations intending to amalgamate.
• Articles of Dissolution (Form 10 under the BCA)
  This form is to be used for voluntary dissolution of a business corporation where the dissolution has been authorized by the shareholders of the corporation.
• Articles of Dissolution (Form 11 under the BCA)
  This form may be used for voluntary dissolution only where the corporation meets all of the following conditions:the corporation has not issued any shares;the corporation has not commenced business; and all of the incorporators or their personal representatives have authorized the dissolution.
• Articles of Revival (Form 15 under the BCA)
  This form is to be used to revive a corporation where the Director has by order cancelled the certificate of incorporation under section 241 of the Business Corporation Act and it is less than twenty years after the date of dissolution
• Ontario Limited Liability Partnership, Extra-Provincial Limited Liability Partnership, Extra-Provincial Limited Liability Company (Form 6)
  This form is to be used to register the firm name to carry on buisness in Ontario. Limited Liability Partnerships (LLP's) is a partnership with special characteristics related to liability and may be formed only by professionals whose governing legislation permits LLP's to practice the profession. Currently in Ontario only lawyers, chartered accountants and certified general accountants may form a LLP.
• Registration of a Business Name for a Corporation (Form 2 under the BNA)
  This form is to be used when a corporation wishes to carry on business under a name different from the corporation's name. Not-for-profit corporations must also complete this form to publicly use a name other than their corporation name.
• Registration of a Sole Proprietorship/General Partnership (Form 1 under the BNA)
  This form is to be used to register a business name for a sole proprietorship (one owner), or the firm name of a general partnership (more than one owner).

• Articles of Reorganization (Form 9 under the BCA)
  This form must be completed and submitted to obtain a Certificate of Reorganization under section 186 of the Ontario Business Corporations Act to amend the articles of an Ontario corporation that has obtained a court order under section 248 of the Business Corporations Act , an order under the Bankruptcy and Insolvency Act (Canada) or an order under the Companies Creditors Arrangement Act (Canada).
• Restated Articles of Incorporation (Form 5 under the BCA)
  This form must be completed and submitted when the directors of an Ontario corporation wish to restate its Articles of Incorporation as amended (i.e. to consolidate all amendments and the original articles into one set of articles). Restated articles supersede the original articles and all the amendments that have been made to them.
• Articles of Arrangement (Form 8 under the BCA)
  This form must be completed and submitted by an Ontario corporation to obtain a Certificate of Arrangement after the corporation has obtained a court order under s. 182(5)(f) of the Business Corporations Act approving an arrangement.
• Application for Authorization to Continue in Another Jurisdiction (Form 7 under the BCA)
  This form must be completed and submitted by an Ontario corporation incorporated under the Business Corporations Act to apply for authorization to continue the corporation to a jurisdiction outside Ontario (transfer out of Ontario)
• Articles of Continuance (Form 6 under the BCA)
  This form must be completed and submitted by a corporation that is incorporated under the laws of a jurisdiction outside Ontario to apply to continue into Ontario as an Ontario corporation subject to the Ontario Business Corporations Act.
• Application for Authorization to Continue under the Co-operative Corporations Act (Form 7.1 under the BCA)
  This form must be completed and submitted by an Ontario corporation incorporated under the Business Corporations Act to apply to be continued as a co-operative corporation subject to the Co-operative Corporations Act.
