What information do I need to provide to change my name?

In your application to change your name, you will be advised of the detailed requirements for your type of change of name application, and the documents that you will need to submit, including:

  • All birth certificates and previous change of name certificates of each person whose name is to be changed. 
  • A statement by a guarantor  who knows the applicant and can confirm that the applicant has lived in Ontario for at least one year prior to the application date. 
  • A marriage certificate, certificate of divorce, annulment certificate (a death certificate of spouse or partner), joint declaration of conjugal relationship. 
  • Proof of notice to your spouse if you are married and are formally changing your name to a different last name (surname).
  • Proof of custody (original or certified copy of a court order or separation agreement) and written consent from all those who have custody, if you are changing your child's name. 
  • Proof that notice was sent to every person who is lawfully entitled to have access to the child.
  • Written consent of the child, if the child is 12 to 17 years old. 
  • The required fee

What if I need to change my name urgently?

People who need their change of name or other information documents right away may be eligible for alternative service options. These circumstances include:

  • Pending adoption
  • Post-secondary graduation
  • Correcting the sex indicated on the birth certificate
  • An immigration matter.

What if I need to travel while my name change is processing?

If you were born in Ontario, the Office of the Registrar General must retain your birth certificate during the processing of your change of name or amendment application (see Service Times).

If you need to travel during this time frame, it is advised that you obtain a passport in your current legal name before submitting your birth certificate with a change of name or amendment application. This way, you may travel using a current passport while your application is being processed.

How do I cancel my name change application?

If you were born in Ontario and have already submitted a change of name or amendment application and need your birth certificate to access services (e.g. a passport), you may choose to cancel your application.
