Post Adoption Birth Information (Adopted Person)

Beginning June 1st, 2009, you can apply for information from your birth and adoption records.

This information is called post-adoption birth information. It includes identifying information, which may include your name before you were adopted and the name(s) of your birth parent(s).

Post-Adoption Birth Information Services include:

  • Copies of adoption orders and Ontario birth registrations for adopted adults
  • Information from adoption orders and Ontario birth registrations for birth parents
  • Disclosure vetoes for adopted adults and birth parents to protect their privacy. Read the section on disclosure vetoes to learn more
  • No contact notices for adopted adults and birth parents who don’t want to be contacted by each other. Read the section on notices to learn more
  • Notices of contact preference for adopted adults and birth parents who know how they want to be contacted. Read the section on notices to learn more

Important Information

  • Post-adoption birth information will include identifying information, like your name after you were adopted
  • If you don’t want your birth parent(s) to know your identity, you must file a disclosure veto
  • If your adoption was finalized before September 1, 2008, you can file a disclosure veto
  • No contact notices and notices of contact preference can also be filed

Applying for Post-Adoption Birth Information

  • You must be an adopted adult who is at least 18 years old
  • Your adoption must have been registered in Ontario
  • Download the adopted person’s guide to Post-Adoption Birth Information and the Post-Adoption Birth Information application form, or call 416-325-8305 or 1-800-461-2156 (TTY 416-325-3408) to get more information
  • This is a free service
  • Send your application by mail, registered mail or courier only
  • Due to a high volume of applications, there is currently no standard processing time and we will not be able to confirm that we received your application

Already Applied?

  • Any applications for disclosure of post-adoption birth information received before June 1st, 2009 will be processed beginning on June 1st, 2009.

More Information

  1. Born Outside Ontario

    • If you were born somewhere else — but you were adopted in Ontario — you can apply for your adoption order only. To get your original birth registration, you will need to contact the province or country where your birth was registered
  2. Disclosure Veto Filed

    • If one or both of your birth parents filed a disclosure veto, we cannot release identifying information relating to that birth parent
    • Instead, you’ll get a copy of the disclosure veto including any statement that was filed by the parent who filed the veto. This statement may include your birth parent’s medical or family history and/or the reasons he or she does not want identifying information released
    • Adopted adults and birth parents have been able to apply for disclosure vetoes since September 2008. Read the section on disclosure vetoes to learn more.
    • Disclosure vetoes are no longer in effect after the person who filed the veto withdraws it, or after the Office of the Registrar General has evidence that the person who filed the veto has died
  3. No Contact Notice Filed

    • If one or both of your birth parents filed a no contact notice, you must agree in writing not to contact the parent(s) who filed the notice before you can receive your post-adoption birth information
    • Once you have agreed not to contact your birth parent(s), you will receive a copy of your post-adoption birth information along with a copy of the no contact notice and any statement which the parent(s) may have filed containing your birth parent’s medical or family history and/or reasons for not wanting contact
  4. Notice of Contact Preference Filed

    • If one or both of your birth parents filed a notice of contact preference, you will receive your post-adoption birth information along with a copy of the notice that tells you how he or she would like to be contacted
