I am an Adopted Person (I was Adopted) - Adoption Disclosure Register

If you want to exchange contact information with your birth parents, birth grandparents or birth siblings, you can apply to add your name to the Adoption Disclosure Register. This could help you get in touch with your birth family.

When you apply, you can tell us the members of your birth family with whom you want to exchange contact information. If those people also apply, we will provide you with their contact information.

How long does this take?

  • Since a match can only be made if your birth relative(s) also apply, a match may take months or even years
  • It is possible that the person you want to contact may not apply

Important Information

  • In voluntarily applying to the Adoption Disclosure Register, you have agreed to the release of your identifying information (like your name and contact information). You will not be contacted to make sure that you are comfortable with this information being released before it is exchanged
  • If you change your mind at any time — or if your contact information changes — you must complete the application to update information or remove name from the Adoption Disclosure Register

Applying to Add your Name to the Adoption Disclosure Register

  • You must be an adopted adult who is at least 18 years old
  • This is a free service
  • Download the guide and Adoption Disclosure Register application, or call 416-325-8305 or 1-800-461-2156 (TTY 416-325-3408) to get more information
  • Send your application by mail, registered mail or courier only
  • We will send a letter of confirmation when we receive your application
