I am a Birth Family Member - Adoption Disclosure Register

If you are a birth grandparent or adult birth sibling of an adopted adult, and you want to exchange contact information with the adopted person, you can apply to add your name to the Adoption Disclosure Register.  This could help you get in touch with him or her.

If you are the descendant of an adopted adult, you are not eligible to apply for the register. However, some community agencies may be able to help you find and contact your relatives.

How long does this take?

  • Since a match can only be made if the adopted adult also applies, a match may take months or even years
  • It is possible that the adopted adult may not apply

Important Information

  • In voluntarily applying to the Adoption Disclosure Register, you have agreed to the release of your identifying information (like your name and contact information). You will not be contacted to make sure that you are comfortable with this information being released before it is exchanged
  • If you change your mind at any time — or if your contact information changes — you must complete the application to update information or remove name from the Adoption Disclosure Register

Applying to Add your Name to the Adoption Disclosure Register

  • The adopted adult must be at least 18 years old
  • This is a free service
  • Download the guide and Application to be named on the Adoption Disclosure Register, or call 416-325-8305 or 1-800-461-2156 (TTY 416-325-3408) to get more information
  • Send your application by mail, registered mail or courier only
  • We will send a letter of confirmation when we receive your application
