On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

pursuant to subsections 7(1), 7(2) and 7(6) of the Local Health System Integration Act, 2006,

Lori Van Opstal of Tillsonburg

be reappointed as a part-time member and re-designated as a Vice-Chair of the board of directors of the South West Local Health Integration Network to serve at the pleasure of the Lieutenant Governor in Council for a term of up to three years, effective November 6, 2016.
