1. The following forms available on the internet at the Ontario Court Forms website are prescribed for the purposes indicated below:

1.  Form 1, titled “Certificate of Lien” (a certificate of lien for the purposes of subsection 1 (1) of the Act), dated November 3, 2021.

2.  Form 2, titled “Certificate of Discharge” (a certificate of discharge for the purposes of subsection 7 (1) of the Act), dated November 3, 2021.

Note: On August 17, 2026, the day section 6 of Schedule 2 to the Keeping Criminals Behind Bars Act, 2026 comes into force, section 1 of the Regulation is amended by adding the following paragraphs: (See: O. Reg. 279/26, s. 1)

3.  Form 3, titled “Security Deposit” (a security deposit for the purposes of section 8.0.1 of the Act), dated July 2026.

4.  Form 4, titled “Surety Reporting Form” (a surety reporting form for the purposes of section 8.1 of the Act), dated July 2026.
