Request for order to comply with part ii
Form of request
1. A request made under subsection 16 (5) of the Act shall be in the form titled “Part II Order Request Form”, as amended from time to time, that is available on the website of the Government of Ontario Central Forms Repository.
Commencement
2. This Regulation comes into force on the later of July 1, 2018 and the day it is filed.
