Birth Certificate: Applying for a Replacement Birth Certificate?

To apply for a replacement birth certificate for a person born in Ontario, there are 4 steps:

  1. Complete the online birth certificate application
  2. Indicate on the form whether you want a small-size birth certificate (short form) or a certified copy (long form).
  3. If you are applying for a birth certificate for an individual nine years of age or older, guarantor information is required. The guarantor is not required to sign your application but must give you permission to provide his or her information.
  4. Submit the form and pay online using a credit card.  Certain Terms and Conditions apply.

You may also complete this form online and print it. Mail or fax the completed form to the Office of the Registrar General or bring it in person to one of our counter locations.
