Change Your Home Address

Change your address for your driver’s licence, vehicle registration, health card and Outdoors Card quickly and easily in one secure, simple transaction.

You can choose to notify one or more of three ministries of your address change – Ministry of Transportation, Ministry of Health and Long-Term Care, and Ministry of Natural Resources.

Your personal information will not be shared among the ministries. Each ministry will only get the information you send to them. Your privacy will be protected as required by the Freedom of Information and Protection of Privacy Act and the Personal Health Information Protection Act.

How soon do changes take effect?

After you finish changing your address online, the information will be updated immediately. Please wait until the day you move to change your address.

What do I need to change my address?

Before you begin, please ensure that you meet these browser requirements.

The Ontario government is committed to ensuring the privacy of your personal information. Please note that some non-standard browsers may retain/cache your personal information. To prevent this, always use the "Next" and "Previous" buttons on the pages, and close your browser to end the session.

Different information is required for each government document that you wish to change your address for.

For your driver’s licence and vehicle registration, the Ministry of Transportation requires your:

1. Driver's Licence Number from your Driver's Licence,
OR
2. Registrant Identification Number (RIN) from your Vehicle Registration,
3. Postal code last reported to the Ministry of Transportation,
4. Complete NEW residential address (where you live),
5. Complete NEW mailing address (where you receive your mail, if different than where you live).

For your health card, the Ministry of Health and Long-Term Care requires:

1. Health Number 
2. Version Code (if applicable) 
3. If you have a red and white health card, your postal code last reported to the Ministry of Health and Long-Term Care
4. If you have a photo health card, the Canadian citizenship or immigration document number you used when registering for your photo health card
5. Your complete NEW residential address (if different than mailing address).

For your Outdoors Card, the Ministry of Natural Resources requires:

1. Your Outdoors Card 
2. Your date of birth
3. Your complete NEW mailing address (where you receive your mail)
