Overview
You can renew your accessible parking permit (APP) if you:
- have a health condition that meets the required criteria
- are a company or organization owning or leasing vehicles to transport individuals with accessibility needs and you meet the required criteria.
You can renew an accessible parking permit if you have a permanent or temporary need for a permit.
If you have a permanent condition
If your regulated healthcare practitioner has certified that the disability is permanent, you do not need to be re-certified by your regulated healthcare practitioner. Renew online or complete only Part A of the application if renewing by mail or in-person.
If you have a temporary condition
If your disability is temporary or subject to change and your accessible parking permit has expired, your regulated healthcare practitioner must re-certify your health condition before you can apply for another permit. This means you must complete Part A of the application form and a regulated healthcare practitioner must complete Part B of the application form.
Once both Part A and Part B of the form are complete, you can apply online, by mail, or in-person at a ServiceOntario Centre.
Renew online
You may renew online if you:
- have an existing permanent accessible parking permit and do not need recertification from a regulated healthcare practitioner
- have an existing subject-to-change or temporary accessible parking permit and have received certification from your regulated healthcare practitioner
- understand and consent to ServiceOntario verifying the information provided in the accessible parking permit application form to determine whether to issue or renew the accessible parking permit
To renew online, you will need the:
- the permit holder’s:
- full name (first, middle and last)
- date of birth
- gender
- mailing and residential address
- phone number
- email address
- current accessible parking permit information
- permit holder’s driver’s licence number (if the permit holder is a driver)
- identity document(s) (if applicable)
- completed accessible parking permit form (if applicable)
Renew by mail or in person
Renew by mail
If you are renewing your subject to change or permanent accessible parking permit by mail, send us:
- a photocopy of your identification proving:
- full name
- date of birth
- signature

If you cannot renew online or by mail, bring your completed accessible parking permit form with identity documents to a ServiceOntario centre.

You may be able to book an appointment ahead of your visit.
Renew for someone else
If applying on behalf of an adult incapable of applying for a permit, you will need to submit one of the following:
- a Power of Attorney document which authorizes you to act on the permit holder's behalf
- a court appointed guardianship proof of Ontario Disability Support Program (ODSP) trustee status
- a Sworn Statement signed by a Commissioner of Oaths (not applicable to dependents under the age of 18)
The authorized representative must also provide both the applicant’s and their own identification documents showing their full name, date of birth, and signature.

Using the permit
An accessible parking permit is issued to an individual person, not a vehicle. You must display your permit with the number and expiry date visible on the dashboard or sun visor of the car to park in an accessible parking space.
Only the person named on the permit can use it to park in an accessible parking space. If a person who is not identified on the permit uses the permit without the permit holder in the vehicle, it can result in fines of up to $5,000 and the permit may be seized.

Interim permit
While you are waiting for your permit to arrive in the mail, you may be eligible for an interim accessible parking permit from your local ServiceOntario centre.
If you are unable to attend ServiceOntario in person and an authorized representative is acting on your behalf, copies of your identity documents and a letter of authorization are required to obtain your interim permit. The authorized representative must also provide their identity documents.

Renew a company vehicle accessible parking permit online
An accessible parking permit may be issued to companies and organizations for vehicles that transport individuals with disabilities, provided they meet the eligibility criteria. You can get a company accessible parking permit for every vehicle owned or leased by your company that is used primarily to transport people with disabilities or owned or leased by your organization to transport people with disabilities on a non-profit basis.
Company permits are valid for 5 years and, if still required, must be renewed every 5 years.
Eligible company
To be eligible, your company:
- must be incorporated and operating in Ontario
- vehicles must be registered to the company
- vehicles must be owned or leased by the company primarily to provide transportation services to persons with a disability
Eligible organization
To be eligible, your organization:
- must be registered and operating in Ontario
- vehicles must be registered to the organization
- vehicles must be owned or leased by the organization to provide transportation services to persons with a disability on a non-profit basis

Acceptable identification
To renew an accessible parking permit, you must provide proof of the following 3 elements of your identity:
- full name
- date of birth
- signature
If applying by mail, include a photocopy of your identification documents only. Do not send original identity document(s).

- Canadian Permanent Resident Card with signature
- Record of Landing (Form IMM 1000)
- Confirmation of Permanent Resident (Form IMM 5292)
- Report Pursuant to the Immigration and Refugee Protection Act with photo (Form IMM 1442)
- Student Authorization/Study Permit (Form IMM 1442)
- Employment Authorization/Work Permit (Form IMM 1442)
- Visitor Record (Form IMM 1442)
- Temporary Resident’s Permit (Form IMM 1442) – formally Minister’s Permit/Extension of Minister’s Permit
- Secure Certificate of Indian Status Card (issued on or after December 15, 2009, by Indigenous and Northern Affairs Canada)
- Driver’s licence, Enhanced driver’s licence or Ontario Photo Card – issued by the Government of Ontario (not expired more than one year)
- Canadian Department of National Defence driver’s licence (Form DND 404)

- Driver’s Licence or Enhanced Driver’s Licence – issued by another Canadian or US jurisdiction
- Ontario Health Card
- Identity card issued by a Canadian or US jurisdiction (enhanced or non-enhanced)
- Ontario Student Card with signature
- Declaration from a Guarantor (Form SR-LD-40)
- Canadian Department of National Defence (DND) Identity Card
Authorized representatives
If an authorized representative is renewing on your behalf, you must sign the application form and provide copies of your identity documents. Your identity documents must prove your full name, date of birth and signature.
If you are unable to sign the application form and an authorized representative has signed the application form on your behalf (not applicable to dependents under the age of 18), the application must be accompanied by one of the following:
- a Power of Attorney document which authorizes them to act on your behalf
- court appointed guardianship
- proof of Ontario Disability Support Program (ODSP) trustee status
- a Sworn Statement signed by a Commissioner of Oaths

- full name
- date of birth

Change permit information
You must update your accessible parking permit when:
- you change your name
- you change your address
- your medical condition changes
- other personal information changes (for example, gender)
Note: You may now apply for a change of address online.
To change your name, medical condition, or other personal information such as gender, you will need to fill out Part A of the accessible parking permit form and provide proof of your change(s). Return a permit
An accessible parking permit must be returned to ServiceOntario within 30 days when the:
- person named on the permit is deceased
- permit holder is no longer a person with an eligible health condition
- permit contains incorrect information
- permit has been cancelled

Lost, stolen or damaged accessible parking permit
Lost or stolen permit
To replace a lost or stolen accessible parking permit online, you will need the permit holder’s:
- full name (first, middle and last)
- date of birth
- gender
- mailing and residential address
- phone number
- email address
- driver’s licence number (if the permit holder is a driver)
To replace a stolen accessible parking permit online, you also need to:
- Visit or call your local police service to report the stolen permit
- Get an occurrence number from the police
- Include the occurrence number on the application
Replace in-person or by mail
Complete these steps to replace a lost or stolen accessible parking permit in-person or by mail:
- Get an application form by visiting any ServiceOntario centre or downloading the application form for an accessible parking permit.
- Complete Part A of the form only (your regulated healthcare practitioner is not required to complete Part B).
- Submit the completed form with identity documents showing proof of full name, date of birth and signature of the permit holder.
If you are applying by mail, include a photocopy of the permit holder’s identity document(s). Do not send original identity document(s).
To replace a stolen accessible parking permit in-person or by mail, you will also need to:
- Visit or call your local police services to report the stolen permit.
- Get an occurrence number from the police.
- Include the occurrence number on the application form.

Damaged Permit
Replace in-person or by mail
You can replace a damaged accessible parking permit in person or by mail only. To replace a damaged permit, you will need to:
- Get an application form by visiting any ServiceOntario centre or downloading the application form for an accessible parking permit.
- Complete Part A of the form only (your healthcare practitioner is not required to complete Part B).
- Submit the completed form with identity documents showing proof of full name, date of birth and signature of the permit holder.
If applying by mail, include a photocopy of the permit holder’s identity document(s). Do not send original identity document(s).

Seized permit
If your accessible parking permit has been seized, you cannot get a new permit until the matter has been resolved in court.
