Learn how to update your address for your Ontario products and services.
Overview
When you move, you need to update your address on various documents such as:
- health card
- driver’s licence
- vehicle permit
- Outdoors Card
- Ontario Photo Card
- OSAP account
- business or corporation records
Many services are available online making it fast, easy, and convenient to manage.
What you will need
- Complete new residential and/or mailing address, including:
- unit or floor number
- postal code
- city
- province
- Other documents as needed
Manage your address with an account
You can create or log in to an account to change your address for the following products
- driver’s licence
- vehicle permit
- health card
- Ontario photo card
- accessible parking permit
Driver’s licence, vehicle permit and accessible parking permit
Driver’s licence and/or vehicle permit
By law, you must update your driver’s licence and vehicle permit within 6 days of your address changing.
You can change your address:
You will need your:
- driver’s licence number or registrant identification number
- issue date on your driver’s licence
If you have moved out-of-province
If you have moved to Ontario from another province or territory find out if your out-of-province driver’s licence is still valid or needs to be transferred.
If you’re moving outside of Ontario:
- return or exchange your driver’s licence to apply for a refund for the remaining time left on your driver’s licence
- check your new province’s requirements for using and transferring your Ontario driver’s licence
Accessible parking permit
You can change your address online.
The other option is to submit Part A of the accessible parking permit application form and proof of your address:
- in person at a ServiceOntario centre
- by mail:

Health card
You must update the address on your health card within 30 days of your address changing, so that the Ministry of Health has your new address on file.
You can change your address:
- online
- in person at a ServiceOntario centre
- mail the completed Change of Address Form to:

Photo health card
If your health card has an address printed on it, you cannot update your address online. You must update it in person at ServiceOntario.
To change the address on your photo health card online, you’ll need:
- your health card number and version code
- the postal code on your current driver’s licence
- the 9 characters found on the back of your health card
Red and white health card
To change the address on your red and white health card, you’ll need:
- your health card number and version code (if applicable)
- the postal code on your current driver’s licence
If you have moved out-of-province
If you have moved outside of Ontario find out how your OHIP coverage is affected.
If you’re moving permanently to another province or territory in Canada, OHIP will cover you for the same services as when you are going away temporarily until the last day of the second full month after you leave Ontario.
Remember to apply for coverage in your new province or territory.
Ontario Photo Card
You can change your address on your Ontario Photo Card:
- online
- in person at a ServiceOntario centre with your Ontario Photo Card
- by mailing a written request to:

- full name
- Ontario Photo Card number
- current address
- new address
Outdoors Card
You can change your address on your outdoors card:
- online
- by phone: 1-800-387-7011
- in-person at a:
Ontario student assistance program (OSAP)
Businesses
If your business has moved, you need to change your address on your ServiceOntario account and with the Ministry of Finance for your business taxes (such as beer tax, wine tax, fuel tax, etc.).
- Change your business address using ServiceOntario account.
- Update your business address for business taxes.
Corporations
You will need to update the address information within 15 days of moving your corporation. To change your address:
- Complete and print the notice of change by an Ontario corporation form.

Cost and Delivery
When you update your address, some Ontario services may need a printed copy of the document with the changes.
