On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

pursuant to subsection 70(2) of the Health Care Consent Act, 1996,

Sandra LeBlanc of Sudbury

be reappointed as a part-time member of the Consent and Capacity Board for a period of two years, effective October 26, 2016 to and including October 25, 2018.
