On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

be reappointed as a part-time Physician member of the Physician Payment Review Board for a period of three years, effective October 22, 2016 to and including October 21, 2019.
