On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant to section 39 of the Local Health System Integration Act, 2006 and section 6 of O. Reg 456/16,

be appointed as a part-time member of the board of directors of the Health Shared Services Ontario to serve at the pleasure of the Lieutenant Governor in Council for a term of up to three years, effective January 1, 2017.
