On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant To subsection 7(6) of the Local Health System Integration Act, 2006,

Janine Van den Heuvel of Troy

be designated as a part-time Chair of the board of directors of the Hamilton Niagara Haldimand Brant Local Health Integration Network to serve at the pleasure of the Lieutenant Governor in Council, effective the date this Order in Council is made up to and including June 29, 2019.
