On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant To subsection 7(6) of the Local Health System Integration Act, 2006,

Lori Van Opstal of Tillsonburg

be designated as Chair of the board of directors of the South West Local Health Integration Network, effective the date this Order in Council is made up to and including November 5, 2019,

And furthermore, Ms. Van Opstal is no longer designated as Vice-Chair of the board of directors of the South West Local Health Integration Network.
