On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant to subsections 7(1) and 7(4) of the Ministry of Health and Long-Term Care Appeal and Review Boards Act, 1998,

Thomas Kelly of London

be reappointed as a part-time member and redesignated as part-time Vice-Chair of the Health Services Appeal and Review Board for a period of five years, effective July 18, 2017 to and including July 17, 2022.
