Privacy statement

This form is intended to capture information, including details on the performance of an inspector or auditor acting in their professional capacity.

Please do not submit any personal information or financial details about your business with your feedback. The term “personal information” has the same meaning as under the Freedom of Information and Protection of Privacy Act, R.S.O. 1990 c. F.31, as amended from time to time. Please note that name, title, contact information or professional designation that identifies an individual in a business or professional capacity is considered as business identity information, not personal information.

Your business information is being collected in order to respond to your inquiry. Your inquiry/ business information will be forwarded to the appropriate ministry(s) for response and will be used to respond to your inquiry. Some of the non-identifying information shared will be used by the Government of Ontario and its service providers to measure website analytics, performance and to improve our services. You will not be placed on mailing lists and your business information will not be disclosed to any third party except for the purpose that we collected this information or as may be required by law.

Ministries of the Government of Ontario are subject to the Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. F.31 (“the Act”), as amended from time to time, and any information provided to the Government of Ontario may be subject to disclosure under the Act. Section 17 of the Act provides an exemption from the disclosure of certain third-party information that reveals a trade secret, or scientific, technical, commercial, financial or labour relations information supplied in confidence where disclosure of the information could reasonably be expected to result in certain harms. Any trade secret or scientific, technical, commercial, financial or labour relations information submitted by your business in confidence in this form should be clearly marked by your business as confidential. A ministry will provide notice to your business before granting access to a record that might contain information referred to in Section 17 of the Act so that your business may make representations to the ministry concerning disclosure.
