Using the provincial online vaccine system

You can use the provincial system to book a vaccine appointment and download your enhanced certificate with QR code.

What you need to use the provincial online booking system

To use the provincial online vaccine system, you need:

  • a green photo health (OHIP) card (you will need numbers from both sides of the card, expired cards will be accepted)
  • an email address and/or a phone number to book your appointment (your own or someone helping you)
  • a modern web browser (the system is not compatible with Internet Explorer)

The provincial online vaccine system will email you a booking confirmation code when you successfully book your appointment.

Please bring this confirmation code with you to your appointment.

Changing, cancelling and missed appointments

If you scheduled an appointment through the provincial online vaccine booking system, you can manage your appointments:

You will need your:

  • health card (information on front and back)
  • date of birth
  • postal code
  • booking confirmation number
  • COVID ID (if you do not have a health card)

If you booked your appointment with the public health unit, pharmacy, hospital, or mobile or pop-up clinic, contact them directly to make any changes.

If you forgot your appointment information

If you scheduled a vaccination through the provincial online vaccine system and forgot when or where your appointment is, call the Provincial Vaccine Contact Centre at 1-833-943-3900. This line is available Monday to Friday (excluding holidays) from 8:30 a.m. to 5 p.m. Eastern time.

You will need your:

  • health card (information on the front and back)
  • date of birth
  • postal code

Downloading your vaccine certificate with a QR code

Vaccine certificates with QR codes

You can download a vaccine certificate with a QR code as a personal record or for certain situations, such as travel.

Ontario no longer requires businesses and organizations to verify proof of vaccination or exemption for patrons to enter their establishment.

Organizations may choose to implement their own vaccination policies or requirements; however, they must follow any applicable human rights, privacy, or other laws, and may wish to obtain independent legal advice that may be applicable in their specific circumstances.

If you don’t have a photo health card

If you have a red and white health card

If you have a red and white health card, you can call the Provincial Vaccine Contact Centre at 1-833-943-3900.

This line is available Monday to Friday (excluding holidays) from 8:30 a.m. to 5 p.m. Eastern time. Information is available in more than 300 languages.

Once your appointment is booked, the contact centre agent will provide you with a booking confirmation code. Please write down this number and bring it with you to your appointment.

If you do not have an Ontario health card

If you do not have an Ontario health card, you are still eligible for the vaccine at no charge and can receive your vaccine certificate.

You can:

  • Call the Provincial Vaccine Contact Centre at 1-833-943-3900.
    • This line is available Monday to Friday (excluding holidays) from 8:30 a.m. to 5 p.m. Eastern time. Information is available in more than 300 languages.
    • This option is available if you fit the following criteria:
      • your public health unit uses the provincial booking system
      • you are able to bring another form of ID for validation
    • You will need to provide:
      • first and last name
      • date of birth
      • Identity verification (when at the vaccination clinic)
  • Contact your local public health unit where you will be asked for another form of identification.
    • The public health unit can:
      • confirm your eligibility for vaccination
      • help book your appointment
      • give you a COVID ID, which you can use to book your appointment or get a copy of your vaccine certificate through the Provincial Vaccine Contact Centre

If you got one or more of your doses outside of Ontario

If you received doses of the COVID‑19 vaccine out of province, contact your local public health unit to record your information and receive proper documentation. They will verify and register your vaccination records in COvaxON and, if you do not have an Ontario health card, provide you with a unique ID code.

You will not be able to get your vaccine certificate until both of your doses are registered.
