Why it’s important to register

You need to register your consent to donate, even if you signed a donor card, to ensure the information is recorded with the Ontario government.

When needed, the Trillium Gift of Life Network will use this record to confirm your consent. They will also reaffirm your consent with your family. In almost all cases, families honour and respect their loved ones’ donation decision if they have evidence that it’s what they wanted.

Registering as a donor is the only secure and guaranteed way to make your decision known.

What you need to register

To register as an organ and tissue donor, you need:

  • to be at least 16 years old
  • to provide your date of birth
  • a health card with your current address on file

Note: Providing an email address when you register is optional. If you do, we will send you a confirmation of registration via email. Your email information is not saved.

To register or update your consent:

To withdraw your donor consent, write and mail a letter including your:

  • health card number
  • full name
  • current address
  • date of birth
  • note saying you wish to withdraw your consent
  • signature
  • date
