How to replace a lost, stolen or damaged driver’s licence.
If you are in Ontario
If you are in Ontario, you will need to replace a lost, stolen or damaged driver’s licence in-person at a ServiceOntario centre. A fee of $35.75 will be collected.
You will need to bring a personal identification document verifying your name, date of birth and signature (for example, a valid passport or a valid photo health card and birth certificate issued by a Canadian province or territory).
You will be given a temporary driver’s licence and your new driver’s licence will arrive in the mail within 4 to 6 weeks.
If you are outside Ontario
If you are outside of Ontario, you can replace a lost, stolen or damaged driver’s licence by:
- submitting a request by mail or fax or
- submitting a request online
You also need to pay the $35.75 fee by either:
- mailing a cheque or money order, or
- mailing or faxing a credit card authorization form

Submit a request online
If you will be away less than six months
If you will be outside of Ontario for less than six months, you can get a one-time, temporary driver’s licence mailed to you if your driver’s licence is valid (not cancelled or suspended). The temporary licence:
- is only valid for a fixed period of time
- will be mailed to your out of province address
Request a temporary driver's licence
A Signature Authorization Form will be ready to download during Step 2 of your online request submission. You can:
- sign and upload this form during your online application or
- send us your completed form or a signed letter by mail or fax
This form must be submitted in order to complete your request. If we do not receive your form within 15 business days, your request will be cancelled.
Once your request is processed, the temporary driver’s licence will be mailed to your out-of-province address indicated on your request within 1-2 weeks.
If you will be away for more than six months
If you will be away from Ontario for more than six months, you are eligible to have your licence replaced with your current photograph and expiry date if your driver’s licence is valid (not cancelled or suspended).
Request a replacement driver’s licence
When you submit your online request, you must also mail or fax us:
- a signed Signature Authorization Form or a signed letter
- a Credit Card Authorization Form, if you are paying by credit card
- a cheque or money order, if you are not paying with a credit card
Your authorization forms will be ready to download during Step 2 of your online request submission.
These forms and payment must be submitted in order to complete your request. If we do not receive your completed forms and payment within 15 business days, your request will be cancelled.
Once your request is processed, the new driver’s licence will be mailed to your out-of-province address indicated on your request within 4-6 weeks.
Submit a request by mail or fax
To replace your lost, stolen or damaged driver’s licence while you are outside of Ontario by mail or fax, submit a signed letter with:
- your driver’s licence number. If you don’t know your driver’s licence number, you can write your full name as it appears on the licence and your date of birth
- the length of time you will be away
- your out-of-province mailing address, telephone number and/or email address

Cost and payment
The cost to replace a lost, stolen or damaged driver’s licence is $35.75.
There is no charge for postage or mailing.
We accept:
- cheque or money order payable to: Minister of Finance (MTO) (for Canadian bank accounts only)
- Visa or Mastercard
We do not accept:
- American Express (AMEX)
- Visa debit
- Mastercard debit

To pay by credit card
To pay by credit card, you must send us either:
- a Credit Card Authorization Form (if you submitted your request online)
- a signed and dated letter of authorization (if you are submitting your request by mail or fax)
The signed and dated letter of authorization must include the:
- name of the card holder
- card type (Visa or Mastercard)
- card number
- expiry date (month and year)
- reason (for example, driver’s licence replacement)
- amount to be charged

Renew your driver’s licence online
If your driver’s licence expires while you are outside of Ontario, you may be able to renew your driver’s licence online.
