• The Geriatric and Long-Term Care Review Committee (GLTCRC) was established in 1989 and consists of respected practitioners in the fields of geriatrics, family medicine, psychiatry, nursing, pharmacology, dietician, emergency medicine and services to seniors.
  • In 2023, the GLTCRC reviewed 17 cases involving 17 deaths and generated 76 recommendations directed toward the prevention of further deaths. Of the 17 case reviews, one resulted in no recommendations.
  • Of the 17 deaths that were reviewed in 2023, the manners of death were:
    • Natural —7 (four male and three females)
    • Accident —7 (three male and four females)
    • Homicide —2 (two female) Note: For the purposes of a coroner’s investigation, the finding of “homicide” does not imply a finding of legal responsibility or culpability.
    • Undetermined —1 (one male)
  • Of the 17 deaths reviewed, eight were male and nine were female.
  • The average age of men whose deaths were reviewed was 83.9 years.
  • The average age of women whose deaths were reviewed was 87.2 years.
  • The average age of all deaths reviewed in 2023 was 85.6 years.
  • In 2023, the most common areas for improvement identified by GLTCRC through their case reviews and resulting recommendations consisted of:
    • Medical and nursing management
    • Acute care and long-term care
    • Communication and documentation
    • Determination of capacity/consent for treatment/Do Not Resuscitate (DNR)
    • Use of drugs in the elderly
    • Use of restraints
    • Education/training
    • Other (for example, quality reviews, research, data collection, referrals to other organizations)
