Overview

If you are an adult 18 years or older, you may receive health care services in a Diabetes Education Program. Diabetes Nurse Educators and Registered Dietitians will help you develop life plans to help minimize your symptoms and delay or prevent the onset of diabetes complications. You will also learn self-management skills from a team of health care professionals.

Services

Diabetes Education Programs provide:

  • diabetes education and support for adults and their families
  • individual and group counselling for patients and family members
  • life plans to minimize symptoms

When to use this option

  • if you are 18-years-old or older
  • if you have been diagnosed with diabetes
  • if your doctor says you are at high risk of developing diabetes
  • if you have a family history of diabetes.
  • if you have a family member who has been diagnosed with diabetes
  • to better manage your diabetes

Referrals

All Diabetes Education Programs are open to the public. Your primary care provider can provide a referral, or you can refer yourself if you do not have a primary care provider.

Appointments

Contact your family doctor, Community Health Centre, Family Health Team or Aboriginal Health Centre or visit Health Connect Ontario to find your local Diabetes Education Program.

Fees

There is no fee for this service. Please bring your health card to your appointment.
