On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 1499/2012 dated October 3, 2012 that reappointed and redesignated

Susan E. Opler of Toronto

as a member and part-time Vice-Chair of the Consent and Capacity Board under the Health Care Consent Act, 1996, be revoked effective as of the date this Order in Council is made.
