Call for applications to expand the Ontario Fertility Program
Learn how to apply to participate in the expansion of the Ontario Fertility Program (OFP), including eligibility, application criteria and timing.
Overview
The province is expanding the Ontario Fertility Program (OFP) to support the delivery of in vitro fertilization (IVF) services for more growing families. This includes:
- increasing funding allocations for existing OFP clinics
- expanding the program to new clinics
The program expansion will:
- cut waitlists
- triple the number of families connecting to government-funded IVF services in hospital and community clinic settings
- ensure patients from across the province can access publicly funded IVF services, including those who live in areas of greatest need, such as northern communities
OFP funding is provided through transfer payment agreements (TPAs) with the Ministry of Health (the ministry) and participating fertility clinics across the province.
Who can apply
To be eligible for consideration as a transfer payment recipient, clinics must:
- be equipped with an on-site embryology laboratory
- be equipped with facilities to deliver high-quality IVF services
- ensure that all oocyte retrievals and embryo transfers performed as part of funded services are performed only at the clinic’s address as specified in the TPA with the ministry
This call for applications is open to all applicants including:
- existing OFP clinics interested in applying for additional IVF funding over and above their current IVF allocation (at the address specified in the TPA)
- new clinics interested in applying to be selected as transfer payment recipients and join the OFP-funded program
- existing OFP clinics interested in applying for funding at an additional site equipped with an embryology lab (a new address not already specified in an existing TPA)
If an applicant operates multiple sites where IVF services are performed, each site is considered to be a separate clinic in the program and would require a separate TPA. Please complete one application form per clinic location.

Clinics that are already participating in the OFP and do not wish to apply for additional IVF funding do not need to complete this application form. Clinics currently participating in the OFP will continue to receive allocations specified in their existing TPAs with the ministry.
Application form
Please note that there are two distinct application forms available:
- one for existing OFP clinics — Application Form for Existing OFP Clinics
- another for new clinics — Application Form for New Clinics
It is essential to complete the appropriate application form that applies to your clinic.
Existing OFP clinics
Existing OFP clinics applying to receive additional IVF funding over and above their current IVF allocation at the address specified in their TPA with the ministry must complete the Application Form for Existing OFP Clinics.
Existing OFP clinics who seek an additional TPA for IVF services as they operate multiple sites and have a secondary site equipped with an embryology lab, or clinics currently receiving funding for intra-uterine insemination (IUI) services only who are interested in applying for IVF funding, must complete the Application Form for New Clinics.
New clinics
New clinics interested in applying to be selected as transfer payment recipients and join the OFP-funded program must complete the Application Form for New Clinics.
How to apply

- Download the Data Release Form (for new clinics and new sites for existing OFP clinics).
- Email the completed application form and, if applicable, the Data Release Form to the ministry at: fertilityprogram@ontario.ca by the submission deadline.
Application deadline
You must submit your application by Thursday, July 24, 2025, at 5:00 p.m. (EST). It is anticipated that clinics will be notified on the outcome of their application in late 2025.
Webinar
An optional webinar will be held shortly after the launch of the call for applications to provide an overview of the OFP-funded fertility program and the call for applications process.
