Health cards, eligibility and coverage

You need a health card to get healthcare services covered by the Ontario Health Insurance Plan (OHIP).

Health cards

Card-holders

  • change your address
  • change your name
  • renew a card
  • switch a red-and-white card to a photo card
  • replace a lost, stolen, or damaged card
  • cancel a health card

First card

  • apply for a card:
    • English
    • French
    • other languages
  • eligibility rules
  • OHIP coverage

Found card

  • found a lost health card

To renew, switch, replace, re-apply or apply for the first time.


Health coverage outside Ontario and for visitors

  • Travelling to other Canadian provinces or territories
  • Travelling outside Canada
  • Longer absences from Ontario
  • Refund claims for out-of-province expenses in Canada
  • Refund claims for out-of-country expenses

Visitors are not eligible for OHIP coverage.
