Overview

Following the one-year anniversary of the September 25, 2023 transition to the Integrated Community Health Services Centres Act (ICHSCA), all centres are reminded to use the updated forms and contact email address for all submissions to the ministry’s Integrated Community Health Services Centres (ICHSC) Program.

Email address

The correct email address is ICHSC@ontario.ca. Please direct communications to this address.

The ICHSC program’s former email address, IHFP@ontario.ca, has been permanently decommissioned and will not process or respond to communications.

Forms

Please use the correct forms found on the ICHSC forms webpage. Using the correct forms will avoid disruptions in service and processing delays.

To access the forms properly, ensure your Adobe software is fully updated.

Any previous forms referencing Independent Health Facilities (IHFs) are considered obsolete and will no longer be accepted.
