The Ministry of Health is requesting that optometrists ensure that banking and address information is up to date

For OHIP payments to be accurately processed and to avoid delays, please update your individual banking information and/or address with the ministry as necessary. Please note it may take up to 30 days to process changes.
If you are currently receiving payments from the ministry directly to your individual bank account, no action is required.
In order to change your banking information, please complete and sign the Application for OHIP Direct Bank Payment for Health Care Professionals form 7698-84. Please submit the form with a void cheque.
Please use the Change of Address for Health Care Professionals form 4885-84E to confirm and/or update your:
- Practice Address
- Mailing Address (if different from Practice Address)
- Telephone Number
- Fax Number
- E-mail address
Both forms may be submitted by email, fax or mail.
