The Ministry of Health is announcing a change of address for the Blended Models Unit.

Effective October 1, 2021 the Blended Models Unit has relocated from:

1075 Bay Street, 3rd Floor
Toronto ON M5B 2B2

To:

438 University Avenue, 4th Floor
Toronto, ON M7A 1N3

This relocation does not change the intake process that was outlined in INFOBulletin 11233 issued on March 24, 2020. All registration related inquires and submission of documents must be made by email only to primarycareinquiries@Ontario.ca.
