Relativity payment and reporting updates on the August 2025 Remittance Advice (RA)

Overview

The Ministry of Health (ministry) and the Ontario Medical Association (OMA) have been working together to implement physician compensation increases in accordance with the Arbitration Award for year 1 of the 2024-2028 Physician Services Agreement (PSA).

Further to Bulletin 250406 — Relativity-adjusted physician payment increases | OHIP INFOBulletins 2025 | ontario.ca updates were made to the OHIP claims system to apply relativity-adjusted (relativity) payments starting on the June 2025 Remittance Advice (RA). Relativity payments will be applied monthly on the same RA the corresponding payment is made (solo or group). The payment will appear as one of the following accounting adjustments:

  • HCP RELATIVITY PAYMENT
  • HCP RELATIVITY PAYMENT OPT-OUT
  • RMB RELATIVITY PAYMENT
  • WSIB RELATIVITY PAYMENT

Updates to individual physician derived specialties

The ministry identified that some physicians had been assigned an incorrect derived specialty. Effective on the August 2025 RA, physician derived specialties and relativity-adjusted rates have been corrected and will continue to be applied to payments until March 31, 2026.

This impacted some physicians who billed using multiple specialties in fiscal year 2024/2025 and/or required further evaluation to determine if they qualified for one of the exception criteria below.   • Family & General Practice
  • Emergency Medicine
  • Critical Care

To determine if you were impacted, you can compare the reported derived specialty on the August 2025 RA Relativity Summary Report to the derived specialty reported on the same section of the June or July 2025 RA to see if it was updated.

For those impacted, payments made prior to the August 2025 RA will be reconciled on a future RA. More information will follow when available.

Payments receiving the global relativity-adjusted rate increase

As per Bulletin 250406 — Relativity-adjusted physician payment increases | OHIP INFOBulletins 2025 | ontario.ca, some relativity-adjusted payments are based on a specific increase rate that is applied regardless of the individual physician’s derived specialty.

It was discovered that the following relativity-adjusted payments were using the individual physician’s derived specialty instead of the specific rate as per the 2024-2028 PSA.

  • Office-based technical fees and Facility Costs for Integrated Community Health Services Centres
  • Community Palliative On-Call Coverage (CPOC) fees
  • Ministry of Children, Community and Social Services (MCCSS) fees
  • Ministry of Attorney General (MAG) fees

Effective on the August 2025 RA, the above payments will now correctly have the global relativity-adjusted rate (9.96%) applied.

Payments made prior to the August 2025 RA will be reconciled on a future RA. More information will follow when available.

Reporting issue

The ministry is aware of additional relativity reporting issues on the Relativity Summary Report of the remittance advice and the OHIP Payment Summary Report impacting reported year to date totals. These issues are actively being investigated. The issues are isolated to reporting and do not impact monthly payments. More information will follow when available.
