Summary of the Relativity implementation and issues in 2025

Overview

The Ministry of Health (ministry) and the Ontario Medical Association (OMA) have been working together to implement physician compensation increases in accordance with the Arbitration Award for year 1 of the 2024-2028 Physician Services Agreement (PSA).

Effective for services provided on or after April 1, 2025, temporary relativity-adjusted payments have been made on the monthly Remittance Advice (RA) for all eligible physician payments for fiscal year 2025/2026.

As of the November 2025 RA all relativity payment and reporting issues are resolved.

Timeline of Relativity Changes

June 2025 RA

  • Relativity payments were implemented in May 2025
  • Most relativity payments were reported and paid on the June 2025 RA for claims submitted with service dates on or after April 1 and received by the May cut-off
  • Relativity payments on premiums and automated payments for the April billing period were not included in the June RA payment

  • Accounting adjustments were processed for the premiums and payments for the April billing period on the July 2025 RA
  • Reporting on the July RA was corrected for physicians’ approved claims totals on their solo and group RAs when physicians bill with more than one group number in a single claims processing period

August 2025 RA

  • Corrections were made to some physicians’ derived specialty
  • Corrections were made to some payments to use the global relativity rate in lieu of the physician’s derived specialty
  • These changes were made for the July 2025 billing period only and were reflected on the August 2025 RA
  • Reconciliation of payments for the April, May, and June billing periods were not included in the August RA payment

  • Reconciliation was completed for the derived specialty and global relativity rate to correct payments for the April, May, and June billing periods
  • The following reporting issues were corrected:
    • YTD relativity payments on the relativity summary report were added. This includes situations where there was a $0 relativity payment in the month
    • OHIP payment summary report was amended to include the relativity payment information for the April 2025 billing period

  • Relativity reporting on the RA was updated to include all reconciliation of previous corrective payments retroactive to the April 1, 2025, effective date

  • Accounting adjustments were completed on the November 2025 RA to correct incorrect October payments
  • The November and October RA relativity reporting now accurately aligns with all relativity payments to date
