Background
System changes were implemented in June 2025 to increase the rates for the following Fertility Fee Schedule Codes (FSCs):
- B600 - Complete Intra-Uterine Insemination (IUI) Cycle
- B601 - Incomplete Intra-Uterine Insemination (IUI) Cycle
- B603 - In Vitro Fertilization (IVF) Cycle Bundle 1
- B604 - In Vitro Fertilization (IVF) Cycle Bundle 2
- B606 - Funded Oocyte Fertility Preservation (FP) Cycle
- B607 - Funded Sperm Fertility Preservation (FP) Cycle
The new rates apply to services rendered on or after April 1, 2025. However, claims assessed before June 2025 were paid at an earlier rate.
Medical Claims Adjustment
A Medical Claims Adjustment (MADJ) has been performed to reprocess claims containing the above FSCs for services rendered on or after April 1, 2025, that were assessed in April or May 2025. These claims have been adjusted to reflect the updated rates.
