New Occupational Medicine OHIP billing specialty

Following consultation with the Ontario Medical Association (OMA), the Ministry of Health ('the ministry') has created a new billing specialty designation under the Ontario Health Insurance Plan (OHIP) for Occupational Medicine, effective April 1, 2026.

Billing specialties are assigned by the ministry when physicians register for a billing number and must be used when submitting claims. To be assigned a billing specialty, the physician must have the appropriate specialty or sub-specialty, as recognized by the College of Physicians and Surgeons of Ontario (CPSO).

The new OHIP billing specialty designation for Occupational Medicine has been assigned number 72 in the OHIP claims system.

Eligibility

To obtain the Occupational Medicine billing specialty, the physician must have a sub-specialty in Occupational Medicine as recognized by the CPSO and reflected as a specialty on the CPSO website in the CPSO Physician Register.

To obtain the CPSO subspecialty, a physician must hold an Occupational Medicine sub-specialty certification from the Royal College of Physicians and Surgeons of Canada (RCPSC).

If a physician does not hold the RCPSC sub-specialty certification but believes they are eligible, they should contact the CPSO directly to assess their eligibility and to update their specialty on the CPSO Physician Register.

Registration process

Register for the new OHIP billing specialty designation by emailing the Provider Registration Services unit at ProviderRegistration.MOH@ontario.ca. In your email, please include:

  • A statement that the “Occupational Medicine” billing specialty designation is being requested;
  • The request start date of the designation (must be on or after April 1, 2026);
  • The physician’s current OHIP billing number; and
  • The physician’s CPSO number.

The Occupational Medicine billing specialty is effective April 1, 2026. Registrations can be backdated to April 1, 2026, or the effective date of the physician’s CPSO subspecialty in Occupational Medicine, whichever is later.

The ministry will verify the physician’s registration with the CPSO and verify that the physician has a subspecialty in Occupational Medicine as recognized by the CPSO. Following completion of this verification process, the ministry will notify the physician by email that the registration is complete, and the Occupational Medicine billing specialty designation 72 has been assigned. The registration date will also be noted in the ministry’s email sent back to the physician.

Submitting claims

Once the registration process is complete and a physician has been assigned the Occupational Medicine billing specialty designation in the OHIP claims system, the physician can begin submitting claims using their new designation. Please note that even with backdating registration, the ministry’s existing stale-date policy for claims still applies.

A physician can hold more than one billing specialty designation, but each claim can only be submitted with one billing specialty designation. When submitting claims, the physician must indicate on each claim which eligible billing specialty designation applies, as appropriate, based on the applicable insured service(s) on the claim(s).

New fee codes

As of April 1, 2026, listings for Occupational Medicine services will be added to the 'Consultations and Visits' section of the schedule. Please refer to INFOBulletin 260304 — Physician Services Agreement related fee schedule code adjustments for additional details.

Only physicians with billing specialty designations in Occupational Medicine (72) will be able to submit claims for payment for services listed in this section (example: the 13 Internal Medicine billing specialty cannot be used to submit claims for services listed in the Occupational Medicine section of the schedule). Any Occupational Medicine fee codes submitted by physicians without the 72 billing specialty will be rejected with error code 'A4D – Ineligible Specialty'.

Any claim submitted with the 72 billing specialty designation must have a service date after the registration date noted in the email to the physician from the ministry. Claims submitted with billing specialty 72 prior to the physician being assigned the billing specialty by the ministry will reject with error code 'EQ2 — 'Specialty mismatch — Specialty Code is inactive or not registered on date of Service'. Claims submitted with billing specialty 72, and a service date prior to April 1, 2026, will also reject with the same error code 'EQ2 - 'Specialty mismatch – Specialty Code is inactive or not registered on date of Service'.
