Hourly rate billing rule update

As outlined in INFOBulletin 260309, FHO hourly payment codes were implemented April 1, 2026. An update to payment processing is being made with respect to claims billed at $0.00.

Starting May 1st, when an hourly code is submitted with a fee billed amount of $0.00 for service dates on or after April 1, 2026, the claim will reject ‘V41 – Incorrect Fee Billed’.

Please ensure that claims for hourly codes are billed at the appropriate dollar value for the number of units being claimed. Please refer to the Hourly rate payment section of the above INFOBulletin for details on how to calculate units and payment amounts.

Physicians with previously submitted claims for hourly rates should verify their May 2026 Remittance Advices to ensure payments are accurate.
